Application for Show Clearance BMI and SESAC License (United States)

This form should be completed prior to your chapter show.
It is forwarded to your District Secretary (or designate) with payment, for calendar approval and processing.

Application for Show Clearance SOCAN License (Form 3016) (Canada)

The SOCAN reporting form (Form 3016) should be completed prior to your chapter show. Section 1 is to be completed before the show and is sent to your district secretary (or designate) for approval and processing. After your show, Sections 2 and 3 are to be completed and sent directly to SOCAN.

Check with your district secretary (or designate) or email chapters@barbershop.org for more information. The fillable PDF may be completed on your computer's PDF viewer, or you can print to fill out by hand.


ASCAP Reporting Form

ASCAP report forms must be completed by a chapter officer and submitted directly to the Barbershop Harmony Society by December 31 of the calendar year indicated on the report form.

After filling out the ASCAP Reporting form, if:

  • no payment is due, please email the completed form to library@barbershop.org.

  • your chapter/district only needs to pay the minimum fee, you can pay the fee online HERE and then email the form to library@barbershop.org

  • the amount due is greater than the minimum fee, please mail a check payable to “Barbershop Harmony Society” and mail together with the completed report form.

ASCAP Concert Fee Schedules

ASCAP Free and/or Benefit Concert Reporting Form

The ASCAP Free and/or Benefit Concert Reporting Form is to be used by chapters and/or districts that host a free or benefit concert during the calendar year. If payment is due, please make payable to “Barbershop Harmony Society” and mail with completed report form.

In regard to free/benefit concerts and BMI/SESAC, please submit the regular BMI/SESAC Report Form and payment to your District Secretary for processing. Fees due are based on the seating capacity ranges, even if admission is free to the attendees.
